The problem is precisely hitting the right measure – that's often quite difficult.
When we struggle with a hangover in the morning, we usually know well that the last drink was already over the line. But in yesterday's social euphoria, it seemed like a great idea!
That's why we won't judge anyone here who occasionally loses control. Instead, let's look at hangovers through the eyes of science – and what to do about them.
What is a hangover? Scientific facts 🔬
A hangover is an unpleasant state caused by the consumption of a toxic substance – alcohol, or more precisely, ethanol (C₂H₅OH).
It is caused by a number of factors – and none of them are pleasant:
1. Toxic effect of acetaldehyde ☠️
The biggest problem: Alcohol does not convert directly into harmless substances in the body. Instead...
Alcohol metabolism:
Ethanol (alcohol)
↓ enzyme ADH (alcohol dehydrogenase)
Acetaldehyde ⚠️ = EVEN MORE TOXIC THAN ALCOHOL!
↓ enzyme ALDH (acetaldehyde dehydrogenase) + glutathione
Acetic acid (acetate)
↓
CO₂ + H₂O (elimination)
Problem:
- Acetaldehyde is 10–30 times more toxic than ethanol
- Damages DNA (potential carcinogen)
- Forms compounds with proteins → immune response
- Causes oxidative stress → cell damage
2. Oxidative stress 💥
During the conversion of alcohol to acetaldehyde, the liver produces:
- Free radicals (reactive oxygen species ROS, reactive nitrogen species RNS)
- Antioxidant depletion (glutathione ↓, superoxide dismutase ↓)
What happens:
Glutathione (GSH) = main liver antioxidant
- Normally protects the liver from toxins
- With excessive drinking → GSH reserves are quickly depleted
- The liver then cannot complete acetaldehyde metabolism
- It must wait for glutathione to be reproduced (8–24 hours!)

Long-term consequences:
- Oxidative stress directly damages DNA in liver cells
- Leads to liver inflammation (hepatitis)
- Progressive liver fibrosis (scarring)
- Cirrhosis (end-stage)
3. Increased inflammation 🔥
Alcohol consumption increases the body's production of:
Pro-inflammatory cytokines:
- IL-10 (interleukin-10)
- IL-12 (interleukin-12)
- Interferon gamma (IFN-γ)
- TNF-α (tumor necrosis factor alpha)

Why does inflammation occur?
- Acetaldehyde forms compounds with proteins (adducts)
- The body identifies them as foreign (similar to bacteria!)
- The immune system reacts → release of pro-inflammatory substances
C-reactive protein (CRP):
- Also rises during a hangover
- Indicator of systemic inflammation
4. Genetic predisposition 🧬
Research has shown:
People who can metabolize ethanol faster have less severe hangover symptoms.
Genetic differences:
East Asian populations:
- Often have the ALDH2*2 allele (mutation in the ALDH2 gene)
- Slower acetaldehyde breakdown → accumulation in the body
- Worse hangover symptoms even after a smaller amount of alcohol
- "Asian flush" = red face after alcohol (vasodilation)
European populations:
- Typically more efficient ALDH2 enzyme
- Faster acetaldehyde breakdown
- Better tolerance (but not safer! – addiction risk ↑)
5. Sleep disruption 😴❌
Alcohol has a paradoxical effect on sleep:
❌ YES, it facilitates falling asleep (sedative effect)
❌ BUT, it devastates sleep quality
What alcohol does to sleep:
- ↓ Shortens deep sleep duration (NREM stages 3-4)
- ↓ Shortens REM phase (rapid eye movement = dream phase)
- ↑ In the second half of the night → frequent awakenings
Consequences:
- Impaired mental performance (attention, memory)
- Reduced ability to concentrate
- Mood disturbances (irritability, depression)
- Hangover symptom intensity correlates with sleep quality

6. Fluid and mineral loss 💧❌
Alcohol blocks ADH (antidiuretic hormone):
- 0.5–1 per mille → ADH completely blocked
- Kidneys stop returning water to the bloodstream
- Increased urge to urinate + ↑ urine volume
- Dehydration
Minerals are lost along with water:
- Sodium (Na⁺)
- Potassium (K⁺)
- Magnesium (Mg²⁺)
Symptoms:
- Weakness
- Fatigue
- Thirst
- Dry mouth
But be careful!
"The impact of dehydration on hangovers is often overestimated. Headaches are more attributable to inflammatory processes, nausea to acetaldehyde, and fatigue to sleep disruption."
Hydration can be restored relatively quickly (a few hours), while hangover symptoms persist for 12–24 hours.
Old wives' tales that DON'T WORK ❌
1. "Never drink on an empty stomach!" 🍔❌
Everyone has probably heard this advice. The problem? It DOESN'T WORK for hangovers.
What's true:
- A full stomach (especially fats + proteins) → slows down alcohol absorption
- = We avoid a rapid onset of drunkenness
- = "Two drinks and we're under the table" won't happen
What's not true:
- Food DOES NOT HELP against hangovers
- Research has shown: Whether we eat before or after drinking, it doesn't help at all
- The only decisive factor is the AMOUNT of alcohol consumed
2. "Drink plenty of water and replenish minerals!" 💧✅ (somewhat helps)
A slightly better idea than food, but not a miracle.
What helps:
- Pure water + isotonic drinks (sports drinks with minerals)
- Fluid and mineral replenishment
What helps a little:
- ✅ No dry mouth
- ✅ Reduced fatigue (minerals)
3. "Alcohol choice is key!" 🥃 (partially true)
True, but not as much as you think:
General rule: The fewer ingredients, the better.
Examples:
- Vodka > Whiskey (study showed milder hangover with vodka)
- Light white wine > Heavy red wine
- Pure alcohol > Sweet cocktails (not due to sugar, but due to quantity!)
BUT:
- Type of alcohol plays a MUCH smaller role than the total amount
- Sparkling drinks (champagne) are not worse (CO₂ bubbles accelerate absorption = faster intoxication, but not worse hangover)
4. "Have a coffee or an energy drink!" ☕❌
Doesn't work.
Study:
- Combination of alcohol + 80 mg caffeine
- After 45 minutes: ↑ alertness, ↑ mood ✅
- After 180 minutes: ↓↓ alertness, ↓↓ mood ❌
= Caffeine only temporarily masks symptoms, then it gets worse.
5. "Have a hair of the dog in the morning!" 🍺❌❌❌
Very bad idea.
Why some people do it:
- Alcohol suppresses some hangover symptoms (central nervous system)
Why it's a terrible idea:
- The effect is only temporary
- The hangover soon returns – and often stronger
- Start of a vicious cycle → increased risk of addiction ↑↑
6. "Garlic soup works!" 🧄✅ (maybe!)
The only old wives' tale that MIGHT work:
Garlic contains:
- Allicin (anti-inflammatory)
- Organosulfides (antioxidants, epigenetically active)
- Ajoene (anti-inflammatory)
Effect:
- Hangover relief (proven in animals)
- Human studies are still lacking, but the mechanism makes sense
